About the Role
This opportunity is for an Enrollment Specialist (Health Outreach) who will serve as an initial point of contact for patients interested in care. The role combines high-volume patient outreach with initial consultations, medical history intake, cognitive health screening coordination, appointment scheduling, and guidance through the early stages of the care journey.
The position requires a patient-first approach, strong telephone communication, accurate documentation, and the ability to work across multiple systems while speaking with patients or caregivers. Responsibilities also include explaining available medical consultations, therapy sessions, and cognitive assessments, assisting with technology questions, and coordinating interactions among patients, caregivers, medical professionals, and external care team members.
What You'll Do
- Contact patients who have expressed interest in care and clearly explain available services and care offerings.
- Conduct initial patient consultations and support comprehensive intake and assessment activities.
- Communicate the value and purpose of medical consultations, therapy sessions, and cognitive assessments.
- Coordinate cognitive health screening campaigns.
- Collect medical history information directly from patients or their caregivers.
- Schedule and coordinate patient appointments.
- Coordinate care-related sessions involving patients, caregivers, medical professionals, and external care team members.
- Assist patients with technology questions and other issues that may affect their ability to access care.
- Document patient information accurately and completely while navigating multiple systems during inbound or outbound calls.
- Follow defined workflows and mandatory scripting requirements consistently.
Qualifications
Required Experience
- Demonstrated call center experience handling high-volume outbound and/or inbound calls.
- Experience working with elderly patients and family members with patience and empathy.
- Experience completing patient documentation accurately and efficiently.
- Experience navigating multiple systems and completing documentation simultaneously during outbound calls.
Required Skills
- Excellent verbal and written communication skills for effective interaction with patients, medical professionals, and internal team members.
- Professional, patient, and caring communication style when working with patients and family members.
- Familiarity with Medicare and insurance concepts.
- Strong typing and patient documentation skills with consistent attention to accuracy and completeness.
- Strong technical aptitude with the ability to quickly learn new software and systems.
- Competency with Google Office Suite.
- Excellent organizational and management skills, including the ability to track complex information flows.
- Ability to quickly learn and consistently follow defined workflows and mandatory scripts.
- Comfort and proficiency working in a remote office environment.

- Previous telehealth experience.
